Getting started
1. Referral
Refer yourself, or ask your health team to refer you. Anyone can refer. Complete the referral form below, or ask your GP, nurse or specialist to send us an email
referral@aotearoadiabetescollective.org.nz.
2. Initial appointment
Book your initial appointment. Everyone new to our clinic starts with a comprehensive one-hour initial appointment ($199). In many cases we order blood tests, and contact your usual healthcare team beforehand. We will also send you a short form to help us prepare and get the most out of your appointment.
3. Ongoing care plan
Choose the ongoing care that fits you. At the end of your initial appointment, we'll make a follow up plan together that suits you best. There's no pressure to continue. If one appointment is all you need, we'll send a clear summary and recommendations to you and your health team. If you want to continue seeing our team we have a range of follow up care options;
Pay as you go:
- Follow up appointment $169
- Quick remote/phone review $50
- Repeat prescription $25
Package of care:
- 3-month package of care $550 (excluding initial apt)
- New insulin pump start $2,149 (includes initial apt)
- Pump transition $550 (includes initial apt)
Subscription care:
- Wrap around care for $20 per week which includes; up to four follow up consults per year, extra remote reviews if required*, access to our on-call nurse 7-days a week and free repeat prescriptions*.

4. Working as a team
We work in partnership with your health team. You can see us once for a specialist assessment and a clear plan for your GP to continue, or you can stay with us for ongoing care while we keep your GP informed with clinical letters at every step.
If you're under a hospital diabetes clinic, you can see us as well; many people use us for nursing, dietitian and technology support and continue to see their usual hospital service. Staying engaged with your GP is a must. We're a specialist diabetes service, not a general practice, and if we're ever concerned about another aspect of your health, we'll contact your care team.
Sweet As! the diabetes podcast
We have recently launched our podcast which is designed for people with diabetes and those supporting them. The goal is to share real stories and cut through stigma and misinformation by providing evidence based education. We encourage everyone to have a listen whether you have diabetes, know someone with diabetes or work with people with diabetes.
